New Patient Forms
Upon scheduling the initial appointment, you will receive intake forms via email to be filled out online. Completion of these forms is required prior to your first visit. Policies are covered in detail on the new patient intake forms.
Insurance & Fees
Insurance: Dr. Almeida-Scaff does not participate in any insurance network, Medicare or Medicaid and therefore all appointments are self-pay. She is considered an "out of network" provider. Full payment for each session is due at the time of service through credit card only. A standard receipt with all of the information needed for you to submit a claim to your insurance plan for out of network expenses (where applicable) will be provided. Please contact your insurance company directly to inquire about mental health benefits and reimbursement rates available to you.
Fees: Please call our office at (954) 799-6604 to discuss appointment fees. Fees vary based on type and length of appointment.
Cancellations and Missed Appointments
When an appointment is scheduled, that time is reserved specifically for you and when it's missed that time cannot be used to treat another patient in need of care. Should you need to cancel or reschedule, please contact the office as soon as possible and give at least 24 business hours notice prior to your scheduled appointment. You can call the office at 954-799-6604 and leave a message or send a patient portal message informing of the cancellation. Missed appointments or failure to cancel your appointment with less than 24 hours notice will result in payment for your appointment charged to the credit card on file.
Emergencies/Crisis Situations
Dr. Almeida-Scaff manages her office alone and therefore you will always interact directly with her. She will make every effort to respond to messages and phone calls promptly. However, due to the nature of her solo outpatient practice, she is not able to provide urgent, crisis or emergency care in a timely manner. Therefore, in the event of an emergency or need of an immediate assistance due to a life-threatening crisis, please call 911 or proceed to the nearest emergency room. For non-urgent matters or for scheduling purposes, please send a patient portal message or leave a message on the office voicemail and Dr. Almeida-Scaff will get back to you within 24-48 hours.
